• We have 3 locations and over 30 Providers and growing
• We provide services in primary care, dental and behavioral health
• 32 patient contact hours, including one telehealth shift
• Patient-Centered Medical Home
• The average daily patient census is 14-20
• On-call is just 4 weeks per year
